Health Center Earns Reaccreditation as Patient-Centered Medical Home
Wood County Community Health Center is one of nearly 1,400 community health centers in the U.S. that provide accessible and affordable medical, dental and behavioral health care services for the entire family and help expand access to care in underserved communities. According to the National Association of Community Health Centers, one in five uninsured Americans, one in three Americans living in poverty and one in five rural Americans are served by community health centers.
In addition to primary care integrated with behavioral health, Wood County Community Health Center provides dental care and offers a convenient on-site pharmacy that helps many people save on the cost of their prescriptions. As a Federally Qualified Health Center, Wood County Community Health Center accepts Medicaid, Medicare and most insurance plans, as well as people without insurance. Many fees are based on household size and income and no one is turned away due to inability to pay.
Wood County Community Health Center recently earned re-accreditation as a Patient-Centered Medical Home with Distinction in Behavioral Health Integration by the National Committee for Quality Assurance (NCQA). This model emphasizes team-based care, communication and coordination, which have been shown to lead to better care and improved management of chronic conditions.
“NCQA Patient-Centered Medical Home Recognition raises the bar in defining high-quality care by emphasizing access, health information technology and coordinated care focused on patients,” said NCQA President Margaret E. O’Kane. “Many times, behavioral health conditions are first identified by a primary care provider, so adding behavioral health care services in a primary care setting is a real opportunity for patients. It knocks down barriers to behavioral care and improves overall health,” O’Kane added. NCQA is a private, nonprofit organization dedicated to improving healthcare quality. NCQA accredits and certifies a wide range of healthcare organizations.
