Private Water Systems Program

Proper maintenance of private water systems is essential for ensuring people who use them have safe water for household use.

The Ohio Department of Health regulates Private Water Systems across the state through statutory authority established under ORC Sections 3701.344 to 3701.347 and OAC Chapter 3701-28. Permitting, inspections and enforcement are conducted by local health departments.

The Private Water Program regulates wells, springs, rain water cisterns, hauled water storage tanks and ponds that provide potable water to fewer than 25 people per day and have fewer than 15 service connections.

This form is required for PWS if you are supplying water to a building, a small business, or multiple dwellings. Additional plans as required in Ohio Administrative Code rules 3701-28-03 (E) and (F). Additional plans are also required for the construction of a pond, spring, or cistern to supply water as a private water system
