Wood County Community Health Center has new patient portal, powered by Intelichart, available for patients now!

The new, user-friendly portal is available to download as a free mobile app via the App Store for all major mobile platforms. To locate the portal, search for Intelichart Patient Portal in the App Store and Google Play Store.

The new portal provides:

• 24/7 access to your medical information, including care plan summaries and lab results
• Request medication refills
• Send a message to your care team
• Online option to request appointments

Save Time with Digital Forms 

In addition to the new portal, patients now have the option to complete paperwork digitally. Forms can be sent to you via text or email in advance of your appointment, simplifying your check-in when you arrive at the health center. Enjoy the convenience and privacy of inputting your own information and check in for your visit from home, the parking lot, or the waiting room.
