Health Department: 419-352-8402 | Health Center: 419-354-9049
Children have different healthcare needs as they grow. We are always here to consult with you about your child’s health and encourage you to bring your child in for preventative care that includes developmental screenings and immunizations.
Have you missed an annual exam or are you experiencing unusual symptoms? Are you ready to start a family but have questions about fertility? Are you concerned that you might have an STD? Do you know about all the types of birth control available?
Our providers offer all recommended preventative screenings. We also offer confidential STD testing and fertility consultations, and you can leave your appointment with birth control and other medications from our on-site pharmacy.
We offer chronic care support for patients with two or more chronic conditions such as diabetes, COPD, high blood pressure, kidney disease, depression or anxiety. Through the program, patients receive assistance in developing a plan for care and participate in regular check-ins to monitor progress.
