To transfer care, please fill out the Release of Information Form found at https://woodcountyhealth.org/wellnesscenter/ReleaseofInformation.pdf or stop by our front desk.

The cost of your medications will depend on several things:

    • Do you have prescription insurance coverage?
      • If yes, then your medication costs will depend on what your insurance coverage dictates
      • If no, then your medication costs will depend on what your annual income is

Any prescriptions prescribed by our in-house providers
The pharmacist will review your specific situation and if appropriate, will request pertinent medical records from the specialist. Once these records are received, the pharmacy will be able to fill your prescription(s).**
