Pharmacy Services

We are a full-service pharmacy for use by our patients. We offer medication affordability consultation including determining what insurance covers, what manufacturer patient assistance programs might they be eligible for, and helping them determine their options. Uninsured patients may be eligible for discounts based on income and household size.

Anyone who receives their primary care at the Wood County Community Health Center
Establishing primary care at the health center is a prerequisite to using the on-site pharmacy. This would require transferring care from your current PCP to the health center. To transfer care, please fill out the Release of Information Form found at http://woodcountyhealth.org/wellnesscenter/ReleaseofInformation.pdf or stop by our front desk.

The cost of your medications will depend on several things:

    • Do you have prescription insurance coverage?
      • If yes, then your medication costs will depend on what your insurance coverage dictates
      • If no, then your medication costs will depend on what your annual income is
  • Have you submitted proof of income to the front desk?

The pharmacist will review your specific situation and if appropriate, will request pertinent medical records from the specialist. Once these records are received, the pharmacy will be able to fill your prescription(s).**

There are several options to explore if you can’t afford your insurance copay(s) at the pharmacy. Please see the pharmacist to discuss your options.
We accept most major insurance companies at the pharmacy, however there are some exceptions. See our pharmacist for more information on accepted insurance plans.
  • In general, hours of operation are Monday, 8:30 am – 6:00 pm, Tuesday – Thursday, 8:30 am – 4:30 pm, and Friday 8:30 am – 2:00 pm.
  • The pharmacy is closed from 12:30 – 1:30 pm for lunch on Monday-Thursday.
  • If you are unsure, call the pharmacy at 419-354-9052. The hours of operation for the week are on the voicemail.
