Clean Plate Award

The purpose of this distinction is to award those facilities who are dedicated to excellent sanitation and food safety knowledge. This award will be offered on an annual basis. Recipients will receive a certificate of excellence, recognition from the Wood County Health Department and may also receive local media coverage.

Applying for the Clean Plate Award is voluntary and completing the application is the responsibility of the licensee. If you think your facility qualifies for the Clean Plate Award, fill out the application and return it to WCHD Environmental Health Department by April 1, 2024.

Criteria

To qualify for the Clean Plate Award, the following criteria must be met by your facility:

  1. The facility must hold a valid Food Service Operation (FSO) or Retail Food Establishment (RFE) license
  2. The current licensee must be the same for at least one licensing year
  3. No confirmed foodborne illness associated with the facility within the past two years
  4. There must be no administrative hearings between the facility and Health Department in the past two years
  5. No confirmed complaints at the facility in the past two years
  6. All applications for annual license renewals must be submitted prior to March 1st deadline
  7. No violation for the following items:
    a) Vomit and diarrheal clean up procedure
    b) Employee health forms on file and verifiable
  8. At least one person per shift that the facility is open must have a certification in an approved ODH food safety training course
  9. Meet all requirements based on your facility’s risk level (I, II, III, IV, or Mobile - check the current risk level on your FSO/RFE license; see the Clean Plate Award application for more details on the requirements based on your facility’s risk level)
